During stacking I've paid attention that something is not right ...

and plus ChrisR's "eggsplosion" - you are both right, my FO lights became dimmer and more yellow. After disassembly appeared that two from four LED cells on the Cree XHP 70 "exploded"

I suspect it happened because of the lose contact in my PWM terminal block - voltage/current spike ...
